On this page you can find the necessary forms required for participation in the SCHS Summer 2025 programs.


  Please be sure to scroll the whole page, as there are a few different forms for Incoming Freshmen from those of Returning Players. Also, all medical clearance paperwork MUST be completed BEFORE being allowed to train or play.

*Note: Should you make the Summer League after camp evaluation, there will be an additional "Summer League" form & $100 donation to cover league needs.

Summer 2025

2025-26 ATHLETIC CLEARANCE INFORMATION

Important

All current players, and incoming Freshmen or new players, will need to complete the Athletic Clearance paperwork and  physicals. 


This must be completed BEFORE you can participate in Summer Camps, Summer League and Placement Sessions.


Don't wait. Get it completed early.


Sea View Orthopedic Medical & SCOR Physical Therapy will be offering physicals on May 17th. This fundraiser is a fast and convenient way to get this completed, while supporting SCHS. (Additional info in flyer).


The athletic clearance forms will be available after May 1st. Click on the ATHLETIC CLEARANCE button to access.
